 App crashes - access violation - NT 4.0/VB 4.0(32 bit)/OLE Automation

1)Application (A.EXE) crashes and a log is generated by Dr.Watson.
        Exception: access violation (0xc000005), Address:....

Enviorment: Windows NT 4.0 (Service Pack 2)

A.EXE is a stand-alone exe which tries to create and manipulate OLE
objects which are available in the same m/c. The log file (drwtsn32.log)
doesn't really help, except that the address specified in the access
violation is associated to oleaut32.dbg.

2)The same application with the same setup runs smoothly under Windows
NT 3.51 (Service Pack 5)

Has any one faced this problem?
