Other than PC/SC (still in flux), there is no standard. It depends on the
reader manufacturer.
And, as a reader manufacturer, we have a quite a few smart card readers
(insert-style, motorized, hybrid mag/smart card, etc...), and some ActiveX
interface software for them (demo code I wrote, but it works well and the
source is free). Makes getting to the card easy, but you still need to know
what commands/parameters you need in order to access the chip (most cards
that claim ISO7816-compliance only support a subset of ISO7816... you need
to know what that subset is).
