I am currently working on a project that includes a form for internet users
to sign up for insurance.  The form includes the obvious things such as
name, address, ect...  It also includes a section for number of dependents.
Instead of just giving the user 10 Name, SS, Age fields to use as they need,
I would rather give them the amount of fields based on the number of
dependents they entered.  I have seen it before but was not able to recall
where. Any help or suggestions are greatly appreciated.
