"A VHDL Primer" by J. Bhasker, 1995, 303 pp., Prentice Hall.

I found a book titled "Structured Logic Design with VHDL" by James R.
Armstrong and F.G. Gray good for beginners.  Its a Prentice Hall
Publication.
