Your example works with FreeBSD, hence it is probably a problem of the
ActiveState port.

OK, I have found a workaround.  Here's the non-working program (ActiveState
Perl only) with the one-liner added:

use strict;
use Tk;
use Tk::FileSelect;

my $MW = MainWindow->new;       # create window
$MW->title('ExportGIF');
$MW->iconify;                   # minimize window

$MW->update; # ****************************************** this make radiobuttons work !

my $filename;

$filename = $MW->getOpenFile(-title => 'Choose', -defaultextension => '.xls', );


{
 my %position;
 $MW->deiconify;

  $position{$filename} = 'nix';
  $MW->Radiobutton(     -text           => 'Position 1',
   -variable    => \$position{$filename},
   -value       => 1,   )->grid(-row => 0, -column => 0, -sticky => 'w');
  $MW->Radiobutton(     -text           => 'Skal ikke placeres',
   -variable    => \$position{$filename},
   -value       => 'nix', )->grid(-row => 1, -column => 0, -sticky => 'w');
  $MW->MainLoop;
 }
