 Finding and listening to ports used by other programs

Hi everybody!

I hope that someone could help me with my problem:

I want to write a program that detects which external programs are connected
to a remote server. Further I want to know which port these progs are using,
what the server's IP-address and port are and what data is exchanged.

Please help!
