She wants a program that tests your knowledge in mathematics. At the
beginning, there will be easy questions. She wants levels of difficulty that
a user can choose from at the beginning, corresponding with the difficulty of
mathematical operations (e.g. addition at level 1, and brackets, exponents,
etc. at the final level, 5).

I didn't see exactly in what you want help. If it's for programming, you better
tell what's your problem. If it's just a level selection, you can make
a program that goes further on complication after each question, and bring
some  entries at differents points of your programm. And if you search for
something that already does that, you 've to look in the directories
'education' of some ftp sites. You'll find something without problems.
