 Macro message everytime I Create a contact

hello,

Newbie question.... I got a message from OUtlook evrytime I load it or
Open a contact witch is using a form that I publish, the message looks
like this :

"This Outlook Session COntain Macro. Macro contain virus it's always
safe to disable macro....." and then got 3 choice, Enable
macro/Disable/Info....

Why I got that message ? I didnt create any Macro !!! how can I get
rite of that message...very annoying for End user using my form !!!

How can I make sure I dont have a macro running ?
